Medication Administration Authorization Form

   Medication Administration Authorization Form
   This form must be completed and signed by a
   parent/guardain and an authorized prescriber for each medication
   to be administered at camp. This includes prescription and over-
   the-counter including vitamins, herbal medicines, and homeopathics.
